Joint Declaration Form (Offline) అంటే ఏమిటి?

Joint Declaration Form (Offline) అనేది EPFO records లో ఉన్న తప్పు వివరాలను సరిచేయడానికి ఉపయోగించే ఒక ముఖ్యమైన form.

ఈ form ని Employee (Member) + Employer (Company/HR) కలిసి sign చేసి EPFO office కి offline గా submit చేయాలి.

ఈ form ద్వారా correct చేయవచ్చు:

• Name

• Date of Birth (DOB)

• Father / Husband Name

• Date of Joining (DOJ)

• Date of Exit (DOE)

• Gender

• EPFO official format లేకపోయినా Plain A4 paper లో submit చేయవచ్చు.
• (Employer seal & signature compulsory)

Required Documents (Attach cheyyali)

Correction type batti documents attach cheyyాలి.

Name / Date of Birth Correction

Any 3 documents attach cheyyandi:

• Aadhaar copy

• PAN copy

• Voter ID

• Passport

• SSC / Marks Sheet

• Bank Passbook copy

Father / Husband Name Correction

• Aadhaar copy (Member)
